Additional/One Time Pay Instructions

  • If one time pay is an initial faculty appointment, it will require the pPAF (Paper Personnel Action Form) and all appropriate backups plus the Additional One Time Pay Form - Faculty.
  • If a faculty member is already employed at 1.00 FTE, a dual compensation form must accompany the Additional/One Time Pay Form - Faculty.
  • If an initial faculty one time pay is in addition to an existing faculty appointment that is less than 1.00 FTE, then only the Additional/One Time Pay Form is to be completed.
  • If a re-appointment on an existing one time pay, only the Additional/One Time Pay Form is to be completed.
  • Forward the original Additional/One Time Pay Form with all necessary backups, from the office of the academic dean to Human Resources.
    Note: If paid through C&G funds, send the original plus one copy from the office of the academic dean to SRAS and SRAS will forward to Human Resources.)
