CLAST PROFICIENCY WAIVER

Minimum Criteria for Consideration:

1. At least four attempts at subtest
2. At least 2.0 average in related courses used to meet relevant Gordon Rule requirement
3. Overall FSU GPA of 2.0 (in good academic standing)

Criteria Considered in Granting:

1. Four true attempts (not Christmas-treed)
2. Demonstrated Effort
   1. Seeking help (documented by tutor statement, Math Help Center, Reading/Writing Center etc.)
   2. Taking extra courses
   3. Progression of scores
3. English as Second Language (ESL instruction documented)

Use CLAST Proficiency Process as Last Resort

1. Associate/Assistant Dean must work with student directly or monitor situation to ensure that every effort has been made to pass CLAST before petitioning for waiver.
2. Paperwork should be checked carefully to ensure that required information is present.

Format for Requesting

1. Memo
   1. Student name, SS#
   2. Specific subtest identified
   3. Demonstrated effort described
2. Attachments
   1. Attachment “D” typed (up to line indicating the date student was interviewed by dean) (see website)
   2. Transcript (with relevant courses highlighted)
   3. Record of attempts (faxed to you by Susan Elliott's office)
   4. Documentation of demonstrated effort to pass exam. Note that documentation must include verification from some source external to the student (e.g. note from Math Help Center or tutor, etc.)
