INSTRUCTIONS FOR COURTESY APPOINTMENTS Anyone with instructional responsibilities at FSU (paid or non-paid) must have an active appointment in the Payroll Data Base. Faculty with instructional responsibilities during times when they are not on the payroll (i.e., summer term or service professors during their off term) must be covered during these periods by courtesy appointments. A new faculty appointment form must be prepared for anyone who is teaching a course who is not being paid by Salary or OPS funds. When you appoint a faculty member to courtesy rank (rank must be held in an academic department) with an “H” or “I” appointment status modifier, the end date should be left blank. It will not be necessary for you to reappoint courtesy faculty each year. It will be necessary for you to terminate the courtesy faculty appointment when the services of the faculty member permanently ends. Please follow the instructions below to complete the FSU Paper Personnel Action Form (pPAF): * We need a New Employee Worksheet for new Courtesy Appointments. Forward the pPAF with appropriate signatures and backup documentation should be forwarded to the Office of the Dean of the Faculties. An original and one copy of the new appointment paper for all courtesy faculty appointments are to be sent from the office of the academic dean to the Office of the Dean of the Faculties. Additional Notes: Please submit the New Employee Information Worksheet for Courtesy appointments. Attach a copy of vita to the appointment paper. If available, also attach a copy of Social Security card for verification. If a person does not have a SS#, i.e., a foreign national teaching overseas or a courtesy appointment in the U.S., call the International Admissions Office (850) 644- 6953 to obtain one. A SS# must be provided even if a SS card is not submitted. Please be sure to enter SS# on the New Employee Information Sheet.
